Between appointments, patients and families are left alone with their questions. Our agents support them every day, set up by their own care team.

The care that

happens between

the visits.

Care needs are growing faster than care teams.

Ageing populations, chronic disease, staff shortages, and paperwork are widening the gap. Patients and families are left alone with their questions between appointments. Our agents support them at home, set up by their care team.

Too much information, too little clarity

Patients and families receive records, results, device data, and health content, often with no one to explain them. The agent turns it into plain language and the next steps their care team set.

Most care happens
at home

Following the plan, changing habits, and knowing when to call for help all happen between appointments. Clinicians cannot be there for this. An agent can, every day.

Set by clinicians, controlled by you

Each agent follows the instructions and warning signs a clinician set, within fixed safety rules. It draws on scientific knowledge and trusted public sources. You decide what it remembers, shares, or forgets.

Personal support,
at low cost

Each conversation costs cents. That makes personal support possible for every patient and every carer, not only the few who ask for it.

Personal Health Agents

How it works for patients and caregivers

Step 1

Step 2

Step 3

Your care team sets it up. After your appointment, your care team sets up an agent for you. They add your condition, your plan, the warning signs, and who to contact.

You scan the QR code. You get a QR code on paper or on screen. Scan it with your phone, and the agent opens in your browser. No app, no account.

You get support every day. Ask questions at any time, in plain language. The agent helps you follow your plan and tells you when to contact your care team.

Personal Health Agent For the person living with a condition

After a consultation, your care team sets up an agent for you. It knows your condition, your plan, and the warning signs your care team chose. It explains what was discussed. It helps you follow the plan, one day at a time. It answers your questions in plain language, at any hour. And it tells you when to contact your care team.

It helps you with:

  • Understanding a diagnosis, a result, or a treatment plan

  • Sticking to your medication, nutrition, or exercise plan

  • Preparing questions for your next appointment

  • Keeping your documents and results in order

  • Knowing when to call your care team

Examples: nutrition counselling, COPD, pelvic floor physiotherapy, medication plans from the pharmacy.

Care Partner Agent
For the people who care

Partners, children, friends, and neighbours do much of the daily work of care. Few of them get any preparation for it. The Care Partner Agent is set up for the carer, not the patient. It explains the condition and what to expect. It helps you plan the week and prepare for appointments. It knows which signs mean it is time to call for help. And it asks how you are doing, because carers need support too.

It helps you with:

  • Understanding the condition and how it may change

  • Planning daily routines, appointments, and medication

  • Preparing for conversations with doctors and nurses

  • Recognising warning signs and knowing who to call

  • Looking after your own health and your limits

Examples: dementia, Parkinson's disease, organ transplant, cardiometabolic disease.
